* Mettre en œuvre des soins de rééducation adaptés pour restaurer ou maintenir l'autonomie fonctionnelle des patients. * Élaborer un plan personnalisé de rééducation, en lien avec le projet de soins global. * Participer aux démarches qualité et aux réunions de coordination des soins. * Optimiser l'utilisation des équipements confiés dans le respect des procédures internes. * Être acteur de l'amélioration continue de l'organisation et des pratiques de la clinique.

Nous recrutons un/une psychologue en activité libérale . * assurer la tenue de groupes thérapeutiques en lien avec les projets de soins personnalisés des patients (expression et gestion des émotions, estime de soi, confiance en soi, réhabilitation psycho social..) au long court * Assurer des suivis individuels au long court * Travailler en équipe pluridisciplinaire * Participer à la traçabilité de la prise en charge sur le Dossier Patient Informatisé
